Description: Adult day health and respite services for seniors and adults with disabilities. Services Include: nursing, rehabilitation, mental health, and support groups. Other services include: case management, ambulation /walking and toileting assistance 
information and referral both within the agency and to community resources, general social work assistance, and on-site foot care. Breakfast and lunch are served (special diets are accommodated). Tube feeding is available upon request. Extended service hours are available, 
Eligibility: Serves adults, 18 years of age and older, with physical, cognitive, social, psychiatric, or emotional impairments who require assistance to restore and maintain independence.

Cost is based on care recipient’s income and is provided on a sliding fee basis. 
Eligibility: Serves people with UNPAID caregivers only. Pre-registration required 
Fees: Sliding fee scale based on client's (not caregiver's) income. Many people have no charge for service.
